I think the box art of the nes games was supposed to represent what the characters "should" look like, given that nes graphics were simple and pixelated
Since Lance was based on Stallone in Rambo, they had to show him with a red bandana in the box art, since that is what he wore in the movie.
Of course later that was retconned in the upcoming games, in which he had the blue bandana, which I guess they did to have more contrast with the red pants.
