I just checked the sprues, and it has 3 head options like I remembered. There's the fully enclosed helm, the... Faceplate(?) Up (with breather mask on below), and a helmet less head.
The two helmet options have near-identical features (the only difference is the faceplate has a different grill pattern; open has a III grill, while the closed has an iron cross grill, and of course the open has no visor).
I have the closed helm on my indom captain, and then I later used the open on my primaris champion, and I think this intercessor has the helmet less head.
