1) Most of the figures that share that helmet use it as a bonus alt helmet. It's an extra piece not needed, but added on for variety.
Also, the Blood Knight and Barbarian Builder got that helmet due to fan demand less so the 4H just giving it to them randomly.
2) Gorgo was a regular Knight, and probably a member of the Order of Eathyron which Ignatius comes from. If they share a similar origin, it makes sense for them to have similar gear. The helmet is less a unique piece of character defining armor and more a generic piece of armor used by multiple troops.
Arethyr is the real big bad of the Arethyr Army, and always has been. Once he comes out Gorgo will be pushed to the wayside.
As for the 2.0 run, *every* single non-troop builder in the KS run had a unique head. Hell, some troop builders came with multiple unique heads (Barbarian Builder, Skeleton, even the Knight Builder has a unique head). We've also seen mid-wave releases use new heads, both Goblins, the Mohawk Dwarf, the Ice Elf, all getting new heads. Hell, I got four generic knights from 2.0 and each one has a unique head.

Yes, there's a lot of head reuses, but most of the time it's optional alt heads or troop builders. There is no lack of unique heads, what we need however is more belts, pauldrons and torso pieces, because THOSE are the parts that start getting stale in a larger collection.
