It was a cardboard wig that that they placed in front of the exposed brains in order to display the doll. The moment the child opened the box she had a doll with brains popping out of it's head, and no actual wig to cover it up with. Also the cardboard wig only covered the brains up from the front of the box. You could still see the brains if you looked at the doll from either side of the box.
