Look at their original orc designs - the "troll" warrior. And look at their elves and hobbits - gnomes, goblins, elves of every flavour would be hairpieces with big ears. And even Chima has similar ideology - all their animals had normal minifigure heads underneath, so you could slip castle armour pieces onto them if you didn't like the giant heads.
This isn't totally universal, though. Some of the creatures, like tauren, would probably be molded (look at the CMF lego minotaur) while others like the draenei would probably get the CMF lego faun legs.
For the orgrimmar grunts, we'd probably get beards or top knot hairs, like we got for the lava dudes in nexo knights. Maybe a wolf hat (Like Akita from Ninjago) for a traditional frostwolf shaman. Honestly, the lava dudes from Nexo knights make a decent band of fel orcs.
