Ive had this issue before but it was caused by a change in pressure inside the blister. Without any sort of suction tool at that size I would start by using a wax or removable glue to try and tease it out. Otherwise you can use a small gauge syringe and use pressure to push it out, you can usually get the needle angled through the back of the card without leaving any major visible mark.

I miss the old Pokemon designs, before they cartoonized everything. I need to check eBay for that old pikachu that would light up in your palm, it had 2 tiny metal bits that used your hand as a conductor... I had a Darth Vader that did that too.
