That's really not the case, Kainé's backstory gets dumped on you upon a second playthrough and in it she's cast out from her village for being different and made fun of for something related to her gender/sex long before the male shade Tyrann inhabits her

You do realize her entire left arm and leg were reconstructed by the shade after she got BTFO right? It's why she's covered in bandages since half of her body is practically a shade at this point and her dick having the shade's color is just an extension of that..
