Almost never. I keep the boxes of all my import figures and just keep the extra parts/accessories in their boxes. For non-imports, I have a tackle box that I put their accessories in.

Rarely ever but recently I somehow lost the strap of the holster to my SHF Mandalorian and it's triggering my tism so much because he's just been standing on the shelf for months and I don't know what could have happened to it. I may have vacuumed it up months ago unbeknownst to me that it was even on the floor.

This is one of the first, maybe the first import figure i ever bought when i was 13, i still have him but basically as the equivalent of a camaro on cinderblocks, I lost every accessory except Rebellion and Ebony and his left arm is so worn down it comes off if yiu adjust it but can stay in poses. And i still fucking love the guy

I keep everything in Ziploc bags. The only time I lose stuff is if something falls off the shelf and its impossibly small, like a cigar accessory or something. Occasionally I've lost stuff in moves though, or to my cats but i usually find it.

How the fuck do people lose accessories? The only- and I mean the only fucking way that is acceptable- is if you're taking a toy with a shit ton of small parts out to play with it in like a fucking field or taking it on a hike and get it out for pictures and then a fucking deer bursts out of the woods and you shit your goddamned pants because what the fuck and you flail around and off goes the mini pistol into the fucking river or some shit.
If your answer is that you lose something just in your house because you have so much fucking shit packed wall to wall and can't even get out the toy without 2 inches of visible floor space to figure out where something went then you're a disgusting fucking hoarder and should light your fucking house on fire with yourself in it.

the only time i lost a bunch of accessories was because i was having sex with my boyfriend and he bumped into my toy shelf and all the figures got knocked out

Same here, lost one of the accessories for my Revolmini Venom Snake because his tiny gun is made up of 3 smaller parts, and I still haven't found all of them.

Haven't lost any in awhile, but the last I remember is the hair from this Mankind fig. He has two removable clumps to emulate when he would pull his hair out and they didn't want to stay in well to begin with, picked him up one day and they were just gone into the ether. Luckily he looks better without them but still wish I could track them down.
