It's a different monster from the original one in Doom 2, where it's a refurbished Cacodemon that spits Lost Souls. In Doom 64, Lost Souls are much faster and much more aggressive, they also explode violently upon death, meaning that they're not just like a seeking missile for the Pain Elemental to use, it means the classic trick of getting up in his face to block his spawns to autokill the Lost Souls is now ill advised, as you will be catching that explosion in your face if you do. He also spits two of them at a time now, and upon death he spawns four of those little bastards, as opposed to the original's three.
Appropriately, they made him much more nightmarish to look at, and gave him some all new and otherworldly sounds. Given that Doom 64 lacks the coolest new monsters from Doom 2 (which I wouldn't want to spoil for someone who hasn't played it but wants to), due to technical limitations, I think the souped up Pain Elemental is a fantastic substitute, he was usually a minor inconvenience at best in the past, but in Doom 64 he's straight up a menace and who can seriously annihilate you if he catches you with your pants down.
