They havent done an international release yet because of the cost, its not cheap maintaining all the bandwidth. They shot themselves in the foot by saying it would be $5 per year, and even more so when they said they would give everyone the first thirty days free. They started the hype train without giving the a thorough rundown of the costs to run everything, and now they're struggling to find a way to handle the will-be influx of data transfers, which could easily cut a hole in their wallet.
Think of if like this, you start a minecraft server, a very basic one, and you start it from your home computer. Lets say, for one reason or another that you didnt set a cap on the amount of users that could play at once. Now you have a laggy server because of hundreds of thousands of connections, which will FORCE you into striking a deal with your internet provider, because you are hogging up all the bandwidth for everyone within x square miles. Now you have to pay overages out the ass and possibly have to pay for new lines to be installed in your part of the town, because of all the bandwidth you're using.
That is whats going on with nintendo. They don't currently have the resources to support that kind of load on their servers. They could rent a bunch of them to deal with the one month influx, but that is only a temporary solution to a permanent problem, which is server load.
The minute they said that it was a yearly subscription and that the first month was free, I was prepared to wait for the delayed release, I knew it was going to happen, I told everyone that it was going to happen, and only a handful agreed with me. Don't get me wrong, i'm pissed that they effectively lied to everyone, but I understand the inherent problem that comes with hosting a server for a bunch of ungrateful brats along with being excited for something new. But that's nintendo, doing new things to try and set a new standard.
