First the Pokéwalker route Yellow Forest replaced the Johto Safari Zone, since Route 47+48 needed *something* at the end of it. Then I added more Pokéwalker locations—completely optional nature-heavy side areas, not inventing major new stuff like towns or characters, just filling in the gaps in the region map with content based on an HGSS novelty.
The most "fanficcy" additions are the Shinto shrine in Ecruteak, and the semi-major plot involving Lawrence III and Shamouti Island. The Shamouti bit is at least based on canon content; I think it's a good use of new music, graphics, map designs, and character cameos. Hopefully the Lawrence III legendary-bird-related plot feels like a worthy sequel to the adventures with Eusine and the legendary beasts. As for the shrine... I just like it. :P Feel free to pretend it's not there.
(I can sympathize with other hacks that start out improving the canon regions, then add "just one" new bit that makes sense in context, because why create a game if you can't *be creative* about it? And then that leads to one more addition, and another, and soon you have an entire original side plot/region
