For example, here's how it was done in oblivion/fallout 3/new vegas. They had to make two outfits for every single outfit, then onwards they made the engine capable of having 1 outfit fit every body type. There's also the marketing aspect of it... "THAT'S RIGHT FOLKS WE HAVE OVER 100 CLOTHING ITEMS!!!!!!!!!" and then it turns out there's only 50 depending on gender. Or how it always turned out, 20 for male and 80 for girls.
