For ball, it's easiest to just use an ordinary Poke Ball, but you can change it. Just don't make it something you can't use like a Dream Ball or Cherish Ball.
Dittos in Black 2 are found wild at level 47, so we'll set level met to that.
Date met, anytime after Black 2 was released. Easiest is to just use today's.
Encounter, put Tall Grass, as shown here. Leave the rest.
