Interestingly, it notes that it's not to be used in the anime. Wonder if they were trying to get ahead of 4Kids' censorship, but ended up not needing to since their contract didn't get renewed in 2005, but didn't want the design going to waste.

idk where it is from though, it's not in the Cutting Room Floor page for Pokémon Learning League.
