People are just taking handful of reaction pics and saving them without bothering to change the filenames. I, for one, have plenty of reaction pictures in my folder with a mixture of changed and unchanged filenames from where I saved them from, but since very few of them are Pokemon-related, most of the ones I've changed the names for aren't going to appear to have their names changed.
Pic related - one in my folder that's been making the rounds recently, with its name unchanged from when I saved it.
