I don't know about the outside visual differences but apparently when you put the game in a DS or 3DS, some other game comes up on the screen though it boots up to the corresponding Pokemon game. From what I had gathered with this, the repros are likely R4 clones that autoboots to the Pokemon rom inside of them.
