I believe that they're created from subjecting Pokemon to extreme psychological, emotional, and physical torture, similar to the Hollywood celebrity MK Ultra conspiracy theory. The reason they never fully reveal this is to keep the game relatively kid friendly. This, of course, is just a theory based on the little information we're given.
