I think I better extension would be "when extended to people who don't reciprocate your empathy by either returning the favor when needed or actively contributing to the community at large".
The main reason altruism has worked for us is that once a tit for tat exchange has been firmly established, there's a ad-hoc insurance mechanism built in that you will be helped, so long as you help others.
