Shuffled teams and items around (making it one of the hardest games in the series in the process, as little as that means in fucking Pokemon), added locations/changed locations that were lacking in the first version, minor story changes, a new battle facility (as gimmicky as the Agency was you can tell it had quote abit of effort put into it, the Evil Team Leader models were even made specifically for it and were only repurposed for Episode RR after they figured it'd be a waste to just use them ehre), new side activities in Photo Club, Warp Ride, and Mantine Surf, etc. It adding outright new mons and not just new forms, a new story scenario in Episode RR, and even more "sidequests" were more extra things it didn't even need to add than essentials to make the purchase worth it.
People acting like USUM added less than what was expected was a mix of it coming out so soon after SM, nostagia-blindness for what the previous 3rd version ACTUALLY added, and expectations being through the roof for it due to speculation on more regionals and more drastic story changes running rampant. If you OBJECTIVELY look at what it did to the base SM laid out it's no more egregious than any other third version
