If a town has no gym it just means they're more or less a suburb of a larger neighboring city. The Elite 4 basically act like an upper assembly of representatives and the League Champion is the head of state. The Leage is also in charge of regulating Pokemon Battles. training, the Pokecenters.etc Under this framework we can conclude that most Pokemon regions run on a form of Meritocracy, since the only way to become a government official is to prove your skills in handling Pokemon.
