The only game it's happened in was Brawl.. 1 is not a pattern. And even then, it's incredibly debateable as to whether or not it even happened.
Data for Roy, Mewtwo and a bunch of others is partially in the game, implying that they were planned to be there from the start.
Personally, i'd be surprised if Lucario was let go, on account of the fact that he's so popular. It wouldn't make logical sense to axe a character who appeals to the masses. Same goes for Ike.
I'm expecting to see both Lucario AND next gen rep (Mewtwo+new form, Victini, Zoroark, whomever..), along with both Ike AND Chrom in Smash 4.
