It's the same for bug pokemon since it's been said that they're easier to evolve. For stage 3 mons I feel like it would take a much longer time, something like several years. Stone evolutions would require the item but also a lot of trust between the pokemon and the trainer. I also feel like it depends on how many pokemon a trainer has. The less pokemon a trainer has the more time can be focused on training that pokemon which means it would evolve sooner.

I live in Canada so I just assumed it worked similar to here. It's free, technically, but we pay through taxes. Medications wouldn't be covered though, but depending on your insurance it could be partly covered.

I feel like the media would have a field day. Eventually it would be normalized and people would move on.

In Paldea it seems pretty influential since it's directly tied to the education system. Maybe once a year they would have an poke-olympics where every country competes.
