They will tell the artist whether the design is good or not, and what they wanted to add to the design variations. But they won't personally fix it themselves, not usually unless the time is really tight and there's too much for the artists to fix themselves.
From my experience, they usually distribute the work and give feedback on what to do (e.g. this character is too good-looking, we should really make her skin a bit darker, etc.) and how to make the art fit with one another. And the rest is the artist's job.
But then again, every studio is different with these kinds of things so I really can't say for certain, but generally, the Art Director vision is how the final game will look.
So, tl;dr. if the game visual looked like shit then it's probably the art director's false.
