Even if you accept their claim that having a subjective experience of gender that doesn't correspond to your biological sex isn't itself a mental illness, being so distressed and impaired in your daily functioning by it that society has to bend over backwards to accommodate your delusions and even then you have almost even odds of killing yourself anyway clearly is.
