No doubt we'll find out in the inevitable lewd follow-up sequence) has had that Pikachu doll since it was a Riolu; it has (presumably) evolved recently and didn't account for its chest spike while hugging the doll, resulting in them puncturing it and getting upset. Their master crudely stitches it up again for them in the last image, but they're still visibly shaken up over having destroyed their precious toy. The tags hint that something else is about to happen later.
