The art that we know was not concept art, but was promotional and as such was made after the games, and sugimori thought he saw a second set of arms when he was making the promo art for all the pokemon and no one from the dev team saw it before it was done. Promo art is what consumers see, and as such the "cannon" needed to be changed to fit the art people expected.
