When any individual looks into a mirror and breaks down into tears, questioning themselves who they are, this is the most common sign of Gender Dysphoria, where the individual knows (at an early age) that they were supposed to be born as the opposite sex. This does not conclude if Spike (from the actual show) suffers this condition, but it is moments like that when people think deeply about what they should have been, baring in mind Spike himself has far more female friends and is not as masculine as he tries to be. This in turn may influence Spike to behave like a girl, but more importantly, it gives Spike the advantage of being or balancing the two.

Spike had admit in Dragon Quest, though he was born a dragon he lives and thinks like a pony. In Sealed Emotions Spike thought about his condition more, not only thinking if he was supposed to be a dragon, but also if he was even to supposed to be a boy, since his attempts in acting like one only makes him look like an oddball among ponies AND dragons. In the story, Spike secretly desires to be a girl, which is where this smooth and elegant appearance took shape.
Dress:
This represents the grace of Spike being a femboy, since he has acted like a girl on several occasions in the show, from dressing feminine to playing with dolls. This only really makes him cute.
