Usually, thought this isn't always necessary, anothet conflict has been introduced before the end of the chapter in order to connect to the next and keep the reader engaged with the flow of the plot. You seem to be ending your chapters where scenes end, and while this might be acceptable for a middle schooler writing silly stories in his notebook, it won't cut it when you're at this level of writing. I apologize if this sounds harsh, but I used to have this problem as well, so I feel a certain bit of nostalgia encountering it again.
I'm not sure how much advice you're actually looking for, but my method is to literally label my Conflict, Climax, and Resolution at the end of each of my chapter documents, so that I can keep track of my own shit. If you're having trouble, this method might help you.
