One 'mon would be the base and have all of the relevant IVs, EVs, moves, etc (with perhaps a situation like Rotom where it could learn a move). The change would happen by equipping it with Venustoisite or something, then picking a Venusaur. Not sure if it'd only last a single battle or what, that'd be up to them.
Honestly though, if we could get things like pic related, I'm down for it.
