I think it's just the early design, where the shell plates are rounded and circular, made worse by bad artwork from that angle, despite Sugimori's strengths. Here's his later US artwork. Then eventually the shell design was updated to be more angular as we know today.

I still have this little blastoise from when I was a kid on my desk. It has a dog bite on its back that I like to think all my blastoise in game have from like an arcanine or something.
