Cain says he has a key, and unlocks it for us. He then says that he'll just leave it unlocked since people will want to go in and fight the gym leader.
However, there's a guardhouse here, meaning that they can unlock it any time they like.
Maybe it's because only key holders are allowed in the Wasteland, in which case why doesn't that guard kick out every tom dick and harry that comes in
