His Masters model also has both hands but there's no visible skin, and according to bulbapedia he holds his right arm behind his back.
In BW intro you can see him using both hands, but the right is dark. No idea if that's a glove or some burn/scar; either way I assume N's coronation happened some time before the start of the storyline and Ghetsis' hand became useless after that, either by some accident/attack or progressive damage.
