I don't want to spoil anything since it's a good twist in my opinion (and it's very understandable to not get to it, since the beginning of the game is brutal), but it ties into the environment of Apocalypse. Because of that, you'll be given a quick rundown of the twist in Apocalypse.
Apocalypse's setting is varied--you're pretty much in Tokyo, though there's a variety of unique areas that you progress through.
