I have a couple
>water slime pokemon
>evolves into ice pokemon if it levels up while frozen
>evolves into vapor pokemon if it levels up while burned
>two different pokemon that are somehow linked (something like separated siamese twins or doppelganger and original, light and dark or something like that)
>can only evolve if both level up during the same battle
