There is Elite Redux, which uses Inclement Emerald as a base. The current version of the game doesn't add any new areas iirc, but there are updates In the works that will overhaul Hoenn by moving things around and adding multiple new areas. It's also going to add day/night system, a season system, and overhauled tilesets.
Something to keep an eye out for.
Asides from that, not many hacks that add new areas to existing regions come to mind.
Dunno about Kanto... there was "Gary's Mod", a hack that let you play through the game from Blue's perspective and access new areas. Sadly, that game was never completed past its second beta.
Johto... you can access a few new areas in the post-game of Glazed I guess, but that's with a sequel story and following an original region of its own.
