You can start the game in Johto or Hoenn by warping to that region and hitting the reset button then New Game. Kanto, Johto and Hoenn do not connect to each other. The only way is by warping. You can set a manual warp to go to any city. Your party, items, and PC carry over between games when you warp. CrystalDust was unfinished so Mt. Silver is not in the game. AFAIK This is also not a catch 'em all hack. You are limited to what you could catch in vanilla FireRed, Emerald and whatever you can get in CrystalDust. (I have no idea if CrystalDust's presence turns this into a catch 'em all or not.)
I am thinking that you could apply a FireRed and Emerald difficulty hack as well as long as all of the map is the same. I don't think you could do anything to make CrystalDust more difficult unless you edit that ROM yourself.
QoL options includes:

I personally prefer the old version as you can use the Esc key to bring it up and the Game Boy L and R buttons to cycle through the menus.
There are some game differences between the old version that's currently on github and the current online version.
