Joy is a female-only Pokemon species. It doesn't matter what the father is; if the mother is a Joy, its offspring will be Joy. The inclination and talent to nurse is their inherent Ability. Occasionally the father passes down an awesome egg move like Surf.

"Nurse Joy" is a title. Each seperate Nurse has their own name (First and Last). It works with the same principle as the Dali Lllama of Pope.

Officer Jenny/'s will work the same.

One of the requirements of being a "Nurse Joy" (which is a title) is that you have to bleach and dye your hair Pink so Trainers can recognize the Nurses even if they're not at a Pokemon center.

Hey now, it's not just that. You can tell the different Joys, Jennies, and Don Georges by certain details! The cross on a Joy's hat, the badge/emblem on a Jenny, and the colored border around a Don George's shirt!

No, the family name IS Joy. The two in OP's picture gave their names as Sayaka and Mai Francois-Joy in the episode. We can infer that males who marry into the Joy family hyphenate their family name with Joy. The English dub changed their names though and didn't preserve this fact.
