It starts out as a normal woman who refuses to eat until eventually her hair takes over forming an extra mouth and movable tendrils that are said to be as strong as steel (hence the Steel typing).
As I may or may not have mentioned in this thread before, Fairy types seem to be normal things (the futa-kuchi-onna is still human) with magic like abilities (it has prehensile hair and an extra mouth) that can't be explained with the usual Psychic typing.
Mawile wouldn't be Steel/Ghost because the futa-kuchi-onna is still a living thing, and it wouldn't be Steel/Psychic because the futa-kuchi-onna doesn't control its hair or extra mouth with telekinesis, telepathy, or any other mind related abilities.
It's Steel/Fairy type because the prehensile hair and extra mouth function just because that's what they do. There is no logical explanation for how a normal person or thing gains or uses those abilities, so it can only be explained away as "magic" - the main point of Fairy type.
