With the use of the Mega Stone (most likely key item) and with the Pokemon holding their respective Mega Item, they evolve into their Mega Evolution. You can revert Mega Pokemon back into their regular form using the Mega Stone. Also - the hold item either provides an added boost to the Pokemon to make it competitively viable or the it disappears when the Pokemon Mega Evolves and reappears when you revert the Pokemon.
