Either Ditto transforms into Blastoise or Transform fails.

Transform doesn't copy items, this includes MegaStones which are needed for MegaEvolution.

I, however, am going to go with (if we're going by characteristics and all), that the result will be a mega Blastoise, as the Ditto will Copy the thing it sees, the Mega Stone is just a formality when you're turning yourself into just 'that creature there'

Maybe it would have the ability of a NORMAL Blastoise, though?

when a Pokémon transforms into a Plate-holding Arceus, it will retain the Arceus's appearance and type, but Judgment will remain Normal unless the transformed Pokémon is holding a Plate itself, as the move copies the Plate's type rather than the holder's

Ditto will transform into Mega Ditto. This is the only way to see Mega Ditto.
