Since the games didn't have a female character until Crystal, they chose to name her Blue after the special edition, while naming the boys after Red and Green versions. If you go by the manga, there is no Leaf. Blue just updates her outfit, much like how there is no Lyra, and Crystal just updated her outfit. Game wise, Leaf and Lyra are correct.
American manga translations for Naming the boys Red and Blue, with Green as the girl was so that it didn't confuse the american audiences. Pokemon was released in america with Blue's updated code, and so we got Blue and an updated Red version. Green was never released to the western audiences.
