So if Tyranitarite is in X and Aggronite is in Y... And Tyranitar is exclusive to Y and Aggron is exclusive to X...
does this mean you can somehow trade items across?

Yes. It was confirmed that you can trade Charizard X's megstone to Y, and vice versa.

It was only transfering pokemon via pokebank that didn't allow you to attach hold items. Regular trading between X and Y is completely normal.
