Those are not permanent, and are used only for a ten second video.
As soon as you get out of the video menu, you'll be back to normal.

i've heard several things from different people all contradicting one another. hair style and color, clothes too. what about skin and eye colors?

When you select your sex you have 3 options. Later in game you can change your clothes (but you must have each "slot" filled, which means you can't get your hat off), your hair colour and your haircut, and your eye colour.
