the player character, so Serena with a hat, if you chose to play her is literally a self insert of yourself, no emotion on her own, just you. So you imagine her as you will yourself, however an NPC has emotion to them and an actual character, sure you can 'imagine them how you like' but you'd literally just have to ignore the character they already are, which is pretty weird.
