I've constantly heard Johto be called things like Kanto's tumor or Kanto's DLC.

So how could it be changed to be a decent region on its own?

HG/SS Kanto was fine as post-game. If you added more, it would practically be its own game (except you start off with high-leveled Pokemon).

Johto was my favorite region. The problem was that they couldn't fit all of Kanto on the GBC cart. That's why Cinnabar blew up and all the caves and forests shrunk.

If they had put everything back for HGSS I would still be playing that game right fucking now.
