How do you breed a Perfect 5/6 IV Pokemon?
Do you just keep throwing the Offspring in with the Parents until you get the right one?

You also involved appropriately IV'd Dittos and such. Here's commonly seen guide.

How do you catch a Ditto with good IVs? I know the Friend Safari only gives 2 Perfect IVs

That's why you need multiple dittos, though it's easy enough to find ones with three through the safari and possible for more with hard work.
