It's based on the first design of a Capumon (later Pokémon) conceived, which evolved into Rhydon—which, as you might know, is also the first Pokémon programmed into R/G/B.

The substitute doll always reminded me of the creature on the statues in gyms in the older games. I think the statues were also in the Ruins of Alph and the Cinnabar mansion among other places possibly.
Lots of guesses could be made about this, but I'm pretty sure >>17992631 is the definitive answer.

They did, it was the generic sprite from pokemon red/blue/green that was used for some pokemon. When you viewed them in your party screen they looked like the substitute. It comes from that.

that's the generic monster type pokemon from the gen 1 party sprite, so really it's either rhydon or a baby kangaskhan

the REAL question is why haven't they started making one for each of the party sprites/egg groups, so the substitutes would look like this

It's Rhydon. Rhydon was the first pokemon officially trademarked as a pokemon, and in the original games' code it's the first mon on the list where pokemon data was stored.

That's why it's the model for substitute and for the statues in the early games.

Make its stats out to be deceptively durable and one of the best walls in the game. Normal/ghost, few attacking options to balance.
