In both diseases the person erroneously perceives themselves, however their misconceptions are entirely in their own head. To compare to my previous analogy, saying you're in the wrong gendered body is just as flawed as saying all the personalities in your head are real people. It's all just a delusion in the minds of the mentally ill.
It's sad to see that one illness is recognized as such and efforts are made to treat and alleviate the illness, whereas the other is paraded around as a lifestyle veiled in malicious lies purported by liberal media.
