We get an app that allows you to visit something like the Sevii Islands that you can play up to 4 players with teams you transfer from your game to the phone. You'd go on a small adventure that could be updated to change every once in awhile.

If anything, I think it would be that device that lets you use amiibo with your 3DS.

However, I think that because people now have the option of buying non-physical copies of the game (and a lot of people do that), they won't want to box stuff with the physical copies and sell it for the same price.

The WiFi adaptor and Pokéwalker were both essentially demos for a new feature in Nintendo's next handheld. I don't expect a new handheld anytime soon, but that's something worth considering.
I think the most likely thing is a mobile internet connection, but I don't think there's any way they could reasonably do that without a massive price for just one game. Doing something with a Smartphone companion app, where maybe you can transfer your team from your game and play on the smartphone, would seem closest to that.

I'm honestly not expecting a physical thing with ORAS, since it would likely be available as an eShop download so some people wouldn't be able to get it.

I expect a 3DS app, remember that the 3DS itself has a pedometer and other stuff so it could do the pokéwalker functions all on its own, meaning you could have a far deeper experience than the stand-alone one.
