Chronologically, when counting all the gym leaders in Gen 1 and 2, Erika is both the 4th and the 20th gym leaders, if counting her inclusion in Gold and Silver. She is also a Grass-type leader.

One of the best ways to beat her is to use fire.

OP is counting Kanto twice. Kanto 8, Johto 8, then she's fourth. But it's still fucked up, because I would say the Kanto order in HGSS is Surge first and then whatever the fuck you want.

Eh, she's the fourth. Most people I would say do Lt. Surge, Sabrina, Misty, and then Erika. But she definitely doesn't have to be fourth (but by that token, she doesn't have to in RBY either).

1) Indigo Plateau is in Kanto. Johto shares the Elite Four with Kanto.
2) You fight the Elite Four after you get 8 badges. If you fight her directly after that, it's 9. There are only 16 gyms in Kanto and Johto. If you count RBY then it's simply redundant.
